ABOUT RESOURCE CONNECTIONS OF PRINCE GEORGES
COUNTY
Resource
Connections began its operation
in July, 2003. The agency is
staffed by professionals who are
enthusiastic and motivated to
provide services to individuals
with developmental disabilities
who live in Prince George's
County. Our name, Resource
Connections of Prince George's
County , sends a clear message
of the value we place on our
role to facilitate connections within the community
and its many resources for the
individuals we serve.
We believe
individuals with disabilities
have the same rights as all
other citizens and should be
afforded the same opportunities
as others.
We encourage
"Self-Determination."
This includes:
The
freedom to make
choices.
The
individual's
authority over the
money it takes to support them.
And
understanding the
responsibilities that come with
making choices.
HOW WE
CAN HELP
Resource
Connections of Prince George's
County. Inc. helps individuals
with developmental disabilities
and their families obtain the
services they need and want so
that they can become as
independent as possible and to
enjoy a good quality of life in
the community of their choice.
Resource
Connections is committed to
building partnerships with the
people we serve, others who are
important to them (family,
friends, etc.) and with agencies
that provide funding or the type
of supports that they want and
need. We attempt to find
cost-effective, responsible and
creative approaches to help
individuals achieve their goals
and dreams.
SERVICES PROVIDED BY RCPGC
We provide a
number of types of Service
Coordination/services to assist
individuals with developmental
disabilities and their families
including
Ongoing Service Coordination:
Case
management (Resource
Coordination) for people
receiving supports throughout
Prince George 's County.
Assisting people in planning for
their lives they choose by
helping them identify their
goals and hopes for the future,
and exploring community
resources that may help them
accomplish their goals.
Advocating for people to help
them access the supports and
services they need
Monitoring the services and
supports people receive.
Gathering feedback from
individuals and families about
their satisfaction with the
services they are receiving.
Identifying gaps in services and
unmet needs and trying to work
on changes in the service
delivery system.
Individual Plans are developed
withal people receiving resource
coordination. The process is
consumer-driven and supported by
the individual's team.
Individuals are supported in
participating in or leading
their own planning meetings.
ELIGIBILITY AND ACCESS SERVICES
Helping
individuals and families apply
for services from the
Developmental Disabilities
Administration
Work with individuals and
families to determine their
needs and preferences
Helping individuals and families
identify those services that
might be appropriate to meet the
identified needs and preferences
Helping individuals and families
identify various generic
community resources that might
assist them
INFORMATION REFERRAL SERVICES
Information and referral
services are provided to the
general public which might
contact us
Resource Connections gathers and
maintains a listing of various
community resources that might
be of assistance to individuals
with disabilities and/or their
families
Staff work in partnership with
other community and provider
agencies to identify gaps in the
service system and unmet needs
and works to develop new and
creative resources
