Developmental
Disabilities
Administration. |

Resource
Coordination
services are
typically understood
as a "case
management" support,
although we strive
not to treat people
as "cases" nor do we
manage people. |

 |
|
Individuals served
are residents of
Prince George 's
County and are
enrolled in the
Community Pathways
Maryland
Developmental
Disabilities
Administration
Waiver. |

|
 |
|
Each
of our Resource
Coordinators
supports
approximately fifty
individuals, helping
them to plan for the
lives they chose by |
| |
|
 |
Building
relationships, |
 |
Assessing
individuals'
wants,
needs
and
desires,
and
|
 |
Connecting
people
to both
professional
and
generic
community
resources. |

|
|
 |
|
Working to assure
that individuals are
able to maintain
connections and
relationships with
those who are
important in their
lives (family
members, friends,
co-workers,
neighbors) is very
important to the
Resource
Coordinators.

|
The overall intended
outcome of our work
with individuals is
to build
partnerships and
create opportunities
to help people
realize their hopes
and dreams. |

Resource
Coordinators provide
advocacy in both a
preventive manner
and in crisis
situation as they
support the
individuals with
whom they are
working. |

consumer-driven

controlled
planning |

Assistance
in
accessing
funding
for
additional
support
needs |

Attending
individuals'
annual
program
planning
meetings
and
other
meetings
called
by or in
behalf
of those
with
whom
they are

Helping
identify
resources |
 |
Responding
to
crisis
situations |

Working
to
ensure
the
development
of a
single
plan
that
both
reflects
an
individual's
desires
and
identifies
the
pathway
or steps
by which
they
choose
to
achieve
their
goals. |
